Terms and Conditions - Digital Home Advantage

This offer is for new residential DISH Network® customers only. Limit one (1) per household.

I acknowledge that I am a new DISH Network® subscriber and I currently do not, nor ever had Dish Network® satellite service. I also represent that my household has never had DISH Network® service. I realize this is for new customers only and if it is found out that myself, or my household, is or ever were a customer of DISH Network®, I will lose any benefits of this promotion.

I agree that the activation address will be the same as the purchase address on the DISH Network® Agreement, which will indicate that I am a first time customer.

I understand that I may receive a DISH Network® Satellite System, including up to four (4) receivers and free standard professional installation at no cost to me. I understand that there are certain minimum requirements that are specific to some of the receivers that I may choose:

  • Displayer 625,722,612,622 – a DISH Digital Video Recorder Service fee of $5.98/month.
  • ViP 622,211,612,722 or Dish222 High Definition Receiver – I must choose a High Definition Programming Pak.
  • DISH 322 or DISHPlayer 625 or HD DishPlayer 622 Dual Tuner Receivers – Must be continuously connected to a land-based phone line to waive a $5.00/month service fee.

NOTE:

Receivers are considered a single receiver for programming charges and DO NOT incur the additional $5.00 charge for an additional receiver when continuously connected to a home phone line (i.e. two (2) DISH 322 receivers for independent viewing on four (4) TV's would count as two (2) receivers for programming charges).

I understand that local channels are not included in the programming and are an additional $5.00/month with the exception of DVR Advantage.

I understand that I must activate my account under the proper "Digital Home Advantage promotion" using the proper "DHA" promotion code. I understand that all equipment installed by DISH Network® remains the property of DISH Network® and must be returned to them if my service should ever be discontinued. If equipment is not returned, I may incur fees ranging from $100 to $425 per receiver, depending on model.

I agree to pay for the service on time when billed by the DISH Network®. A service charge of $5.00 will be added to late payments. Bills not paid after 30 days will constitute a suspension of service and/or equipment charges.

I understand that there is a limit of one (1) complete system per customer.

I understand that since satellite dishes are installed externally on my residence, if I rent an apartment, condo, townhouse, or live in a rental community, I will have written permission from my landlord or homeowners association available at time of installation. I understand that this offer is subject to credit acceptance by DISH Network®, and therefore my Social Security Number (SSN) and major credit or debit card is required. The SSN and credit card provided must match the name of the individual receiving the equipment, the FREE basic professional installation, and who will be subscribing to the DISH Network® programming package. When DISH Network® verifies my credit, any discrepancies in name, address, credit card or SSN provided may cause DISH Network® to immediately reject my claim.

Programming and Equipment Promotions

PROGRAMMING OFFERS require participation in Digital Home Advantage with 24-month commitment. After promotional periods, customer must call to downgrade or then-current prices will apply. 12-Month Programming Credit: Requires subscription to qualifying programming. Customer receives a credit for each of the first 12 months. Credit amount will depend on programming package selected. 3-Month Premium: Customer receives a credit for each of the first 3 months. Digital Home Advantage: Requires 24-month qualifying programming purchase. Social Security Number, valid major credit card and credit approval. If qualifying service is terminated prior to end of 24-month period, a cancellation fee equal to the lesser of $360 or $15.00 per cancelled month of service will apply. Equipment must be returned to DISH Network upon termination of qualifying service. Limit four (4) tuners per account. Monthly package price includes an equipment rental fee of $5.00 or $7.00 for the first receiver, based on selected model. A monthly equipment rental fee of $5.00 or $7.00 will be charged for each receiver beyond the first, based on selected model. A $5.00/mo. TV2 receiver connection fee applies for each receiver; fee will be waived monthly for each such receiver continuously connected to Customer's phone line. HD programming requires HD receiver and HD television (sold separately). Customer must subscribe to HD programming or a $7.00/mo HD enabling fee will apply. Lease upgrade fee will apply for select receivers (based on model). Then-current prices will apply for all programming and equipment fees. Offer ends 1/31/10 and is available in the continental United States for new, first-time DISH Network® residential customers.

All prices, packages and programming subject to change without notice. Local and state sales taxes may apply. Where applicable, equipment rental fees and programming are taxed separately. All DISH Network® programming, and any other services that are provided, are subject to the Terms and Conditions of the Promotional Agreement and Residential Customer Agreement, available at www.dishnetwork.com or upon request.

Local channel packages by satellite are only available to customers who reside in the specified local Designated Market Area (DMA). Local channels may require an additional dish antenna from DISH Network®, installed free of any charges with subscription to local channels at time of initial installation. Social Security Numbers are used to obtain credit scores and will not be released to third parties except for verification and collection purposes only or if required by governmental authorities. Digital Protection Plan

The DISH Network® Digital Protection Plan is provided free of charge for 9 months, to those who agree to a 24-month programming commitment.

The Digital Protection Plan provides the following:

  1. Priority Technical Support (24/7 Toll Free Service Number)
  2. Free Replacement equipment (includes shipping and handling)
  3. Cabling Repairs
  4. Reduced price for in-home service calls ($29 fee instead of $99)
  5. Dish Movers Program - DISH Network® will perform a free standard professional installation in your new home if you move, up to one time per 12 month period.

The Digital Protection Plan is free for 9 months with a 24 month programming commitment. It is available for an additional $5.99 per month after the first 9 months, or without a 24 month programming commitment.

Local TV

Local TV channels are offered only in specified areas and may be provided through an outdoor antenna or a local dish antenna. Off-air broadcast picture quality may vary based upon location. Local Broadcast Networks by satellite are only available to customers who reside in the specified local Designated Market Area (DMA).
